A chain of 6 outpatient clinics across Delhi-NCR was struggling with paper-based patient intake. New patients filled in handwritten forms at reception, which staff then manually re-entered into the clinic software, a process that took 18–22 minutes per patient and was a major bottleneck during peak hours. Illegible handwriting caused data entry errors, and there was no way for clinicians to preview patient history before the consultation. The manual process also made ABDM compliance difficult.
HMB replaced the paper-based intake process with a fully digital pre-visit workflow. Patients receive a WhatsApp/SMS link 30 minutes before their appointment and complete their intake form on their phone. Data flows directly into the clinic system, and the doctor sees a pre-populated patient summary card before the consultation begins.
